Official Transcript Request Form
If you would like to request an Official Transcript, please print, fill-out and mail the Transcript Request form.- Include a payment of $10.00 per transcript request with your Transcript Request Form.
- Allow 5 to 10 days for processing.

Populi Student Portfolio User Guide and Self-Assessment Forms

Students are required to compile portfolios of their work throughout the course of their program of study. The purpose of the student portfolio is to assist students in evaluating their progress towards achieving the intended Student Learning Outcomes for their program and to ensure that each student successfully completes the graduate degree. The student portfolios also assist the faculty in the ongoing assessment of the Seminary’s academic degree programs and its documentation and evaluation of each individual student’s learning and progress.

Self-Assessment Forms can be completed online by downloading the form, completing the form, and "'Saving As" with your name and short title of paper for easy identification. Then add the form accompanied by the paper to Populi. Please choose the self-assessment form that coincides with your degree program.
